Aims:

The HACC Program delivers high quality, affordable and accessible services in the community that are essential to the well being of our elders, younger people with a disability, and their carers.

The Program aims to support these people to be more independent at home and in the community, and reduce the potential or inappropriate need for admission to a residential care facility.

The Program is a key element of the Australian Government’s aged-care policy and its vision for a world-class community care system.
The HACC Program provides services such as domestic assistance, personal care as well as professional allied health care and nursing services, in order to our Elders, younger people with a disability and their carers to be more independent at home and in the community and to reduce the potential or inappropriate need for admission to residential care.

We also provide one-on-one support for young people including listening up to what they want, working with them on their goals and speaking up on their behalf to others. We also can provide case management when necessary. We work with all our youth to give advice and help with educational and career pathways when and if they want it.

We love to organize and take part in camps as much as our young Koorie community members do and we look forward to hearing from you about how we can continue to create more opportunities for our young people.

Planned Activity Group (PAG)

This service aims to assist the frail aged, people with a disability or socially isolated citizens by providing a day in a supportive atmosphere where they may join together with other citizens for socializing and recreation.
